MEMBERSHIP Cancellation Request Form


We ask for an advance notice of 30 days so that we can properly terminate your membership. This will ensure that our system does not charge your account. Our system and staff cannot base your desire to cancel your membership purely on your attendance. As such, please communicate your membership cancellation via this cancellation form. You will receive a written confirmation once your membership has been cancelled/terminated. You can see this policy explicitly stated in our Membership FAQ section as well.
